We are required to have liability insurance, which requires a liability and consent for emergency treatment form for everyone who will be present at Hyde Park CoC, regardless of whether or not they are participating in a class. Those can be filled out online here. The facility rules referred to in the form can be accessed here.

Some events we travel to require a signed parent permission form. Print out and complete any that apply. Then either leave the signed copy in the mail bin at the bottom of the stairs, or scan and return to me.
